Completing forms on behalf of a resident is a task that should be done by the NCOPE-recognized mentor that the resident most often works with.<\/p>\n

    \n
  1. The resident should be assigned to the mentor or director who needs to complete the form.\u00a0 If the resident is not linked to the mentor wishing to complete a form, the resident must submit a mentor change request<\/a> to the NCOPE Staff first.<\/li>\n
  2. Once the resident is assigned to the correct mentor, the mentor should sign in to NCOPE Tracker by visiting https:\/\/ncopetracker.org\/admin<\/a> and clicking on “Forms Tool” as illustrated below.\u00a0<\/li>\n<\/ol>\n

    The mentor should then select the the resident they wish to complete the form on behalf of using the pull-down menu and then click “View Survey” which may open the survey in a new tab depending on how your browser is configured.\u00a0<\/p>\n
