It is the responsibility of the residency program director and resident to maintain records.  Copies of each form are emailed to the address the resident uses to log in as well as their assigned mentor.  While you can run the Resident Status Report anytime to see the forms NCOPE has received, copies of the completed form as a PDF should be independently maintained by the personnel at the residency site.  In the event the resident and the mentor/director has deleted all emails and lost all files, they may make a request to the NCOPE residency staff for copies by providing the name of the form, the quarter the form was submitted, the name of the resident, and the name of the residency site.
