Completing forms on behalf of a resident is a task that should be done by the NCOPE-recognized mentor that the resident most often works with.

  1. The resident should be assigned to the mentor or director who needs to complete the form.  If the resident is not linked to the mentor wishing to complete a form, the resident must submit a mentor change request to the NCOPE Staff first.
  2. Once the resident is assigned to the correct mentor, the mentor should sign in to NCOPE Tracker by visiting https://ncopetracker.org/admin and clicking on “Forms Tool” as illustrated below. 

The mentor should then select the the resident they wish to complete the form on behalf of using the pull-down menu and then click “View Survey” which may open the survey in a new tab depending on how your browser is configured.
