Completing forms on behalf of a resident is a task that should be done by the NCOPE-recognized mentor that the resident most often works with.
- The resident should be assigned to the mentor or director who needs to complete the form. If the resident is not linked to the mentor wishing to complete a form, the resident must submit a mentor change request to the NCOPE Staff first.
- Once the resident is assigned to the correct mentor, the mentor should sign in to NCOPE Tracker by visiting https://ncopetracker.org/admin and clicking on “Forms Tool” as illustrated below.
The mentor should then select the the resident they wish to complete the form on behalf of using the pull-down menu and then click “View Survey” which may open the survey in a new tab depending on how your browser is configured.
