How Do I Access Evaluation or Competency Forms for a Resident?

1. If a mentor has an NCOPE Tracker account and a resident is linked to them, they will have the ability to submit evaluation and competency forms for the resident.

2. The mentor must successfully log in to NCOPE Tracker.  The mentor login for NCOPE Tracker is accessible by visiting: https://ncopetracker.org/admin 

3. The mentor can see if they have a resident by viewing their dashboard, which is the first screen visible after accessing NCOPE Tracker.  If the mentor has “0 Active Residents” then the resident must request a mentor change, which only the resident can trigger.  CLICK HERE to view the tutorial on changing a mentor.  Assuming the mentor has at least one active resident, they can access the form by clicking on the “Forms Tool” as shown below.

4. Once the mentor selects the forms tool, the screen below will become visible.  Identify the form you wish to complete, and then, using the pull-down menu, choose the resident for whom you want to submit a form.

5. The form will open.  Depending on your browser it may open in a new window/tab.  You should see your name and the resident’s name listed at the top of each form, as shown below.
